CASE MANAGEMENT PROGRAM FOR THE FRAIL ELDERLY
What is Case Management?
Case Management is a mechanism for assuring that an individual's needs are met by coordinating and linking the components of a service delivery system. The goal is to provide effective and efficient service to the client.
Case Management depends on a multidisciplinary approach to assessment and problem solving; it includes the following:
- a comprehensive assessment of the needs, strengths, and resources of the client.
- a problem oriented and goal directed plan of care, developed by the team and including the client when possible.
- clear understanding by the service providers and the client as to what will be provided, when, how, and by whom including the responsibilities of the family or informal service providers.
- reassessment and restaffings as needed to monitor the situation, evaluate the effectiveness of the services, and make adjustment to the case plan as needed.
- the ongoing assistance of a case manager.
The Case Management Program for the Frail Elderly is a cooperative program with community organizations and the Iowa Department of Elder Affairs.
ELDERLY WAIVER SERVICES
The Elderly Waiver Program is a Medicaid Program for home care options with case management.
Persons who meet all of the following criteria are eligible for elderly waiver services:
Age 65 or over
In need of nursing home level of care
Iowa resident
Meet income and resource guidelines
