Helping Seniors Stay In Their Own Homes
There may come a time when you feel you can’t manage your home and health needs on your own. If you want help, Generations is there! The Case Management Program for the Elderly can coordinate services to help you stay safely in your own home. To qualify, you need to be at least 60 years old, live in Iowa, and have a need for long-term care services.
What Is Case Management?
It is a state program that coordinates services that will help you live safe and comfortable in your own home. Case Management will not take the place of family, friends or neighbors who are already helping you. It will support their efforts and fill in where needed. You want to stay in your own home and we want to help you. Case Management represents a lot of people and agencies who want to help support you in your home.
How Does It Work?
A home visit will be arranged by a Generations Case Manager. She will ask some questions and determine which services meet your needs. You choose the services you want and the Case Manager will arrange and continue to coordinate these services for you. She supports your well-being and responds to your needs.
What Does It Cost?
There is no charge to participate in Case Management; however, there may be a charge for the services that you choose. Many service providers may use a sliding fee based on your income. Some costs may be covered by Medicare and private insurance. Or you may be eligible for the Medicaid Elderly Waiver, which covers the cost of many of the services. You also have the option of paying toward the cost yourself.
How Can It Help Me?
There are over 85 providers that can give you the help that you need. You may choose one or many different services. A typical care plan may include home-delivered meals; help with house cleaning and transportation to doctor's appointments. Or it could be as simple as installing a personal emergency response-system. It's your choice as to the amount of help you get and by whom.
When Should I Consider Case Management?
- Personal needs cannot be met by family and friends.
- Multiple health problems cause a need for assistance with daily tasks.
- Alzheimer’s disease, severe memory loss or other forms of dementia require extra care and help.
- A nursing home placement seems to be the next step.
- Help is needed after returning home from a hospital or nursing home stay.
I've Heard About The Medicaid Elderly Waiver Program. What Are The Benefits?
Applications are submitted to the Department of Human Services and are based on health and income levels. If you qualify, you may receive:
- A Title XIX card which pays for dental, eyeglasses and other health needs.
- Service providers will be paid for services you may need, such as:
- Homemaker service
- Transportation
- Home-delivered meals (includes liquid supplements)
- Assistive Devices (canes, walkers, bath chairs, etc.)
- Chore Services
- Nursing services
- Home and vehicle modification
- Home health aide
- Mental health outreach
- Nutrition counseling
- Personal emergency response system
- Consumer Directed Attendant Care (enables a family member to provide different services)
- Adult Day Care
- Respite Care
